Find out the facility's typical response time to call lights during the night shift specifically, and whether this is tracked. Some facilities can provide data on this; a facility that can't or won't answer at all is worth treating with more caution.
Ask about fall-prevention measures specifically for nighttime
Ask what specific measures are in place for residents at higher risk of falling overnight — bed alarms, low beds, regular overnight checks — and whether these are individually assessed rather than applied as a blanket policy. A good facility tailors overnight fall prevention to each resident's specific risk profile.
Ask how falls are recorded and followed up
Ask how the facility records falls, whether family is notified when a fall occurs, and what happens afterward — a fall-risk reassessment, a physician review, adjustments to the care plan. A facility with a clear, consistent process is telling you something different from one that treats each fall as an isolated event.
Cross-check against CMS data
CMS Care Compare publishes staffing data based on payroll records, which can be compared against what the facility tells you directly. A meaningful gap between the payroll-based data and what you're told on a tour is worth asking about specifically.
