Systemic vasculitis can affect very different organs, so the diagnosis alone does not describe the residential-care workload. One resident may need renal and blood-pressure monitoring; another may have neuropathy, skin lesions, lung risk or treatment-related infection. Symptoms can also be subtle in an older person.
The Italian RSA needs an organ-specific baseline and a route to the specialist team. It should not treat every fever as relapse or every new weakness as ordinary ageing. Create an organ-by-organ risk map
Name the vasculitis diagnosis, current activity, previous relapses and organs affected. Record renal function and urine findings if relevant, lung history, neuropathy, skin changes, eye or ear involvement, cardiac risks and prior thrombosis. Include the specialist centre and the most recent assessment.
Translate this into function: balance, foot sensation, hand use, fatigue, vision, hearing, continence and assistance required. The facility can then assess staffing, equipment and escalation needs instead of accepting a generic “stable autoimmune disease.”
Make immunosuppression operationally safe
List each immunosuppressive medicine, dose, schedule, indication, last administration and monitoring. Include steroids, biologic infusions, prophylactic treatments and vaccination advice. Mark medicines that cannot be stopped abruptly and the person authorised to adjust them.
Confirm ordering, storage and administration, especially for intermittent injections or infusions. Staff should recognise treatment adverse effects and interactions rather than attributing every symptom to vasculitis. Record who reviews results and how the RSA obtains urgent specialist advice outside a routine appointment.
Schedule monitoring with ownership and thresholds
A list of blood tests is incomplete without due dates, responsible clinician and action levels. Build a calendar for blood count, renal or liver tests, inflammatory markers, urine, blood pressure and other disease-specific checks. State where samples are taken and how results reach the reviewer.
Ask what happens when a test is missed, haemolysed or abnormal. The overseas family should not become the result-tracking system. The RSA needs a documented loop from request to collection, review, action and care-plan update.
Distinguish infection, toxicity and relapse
Fever, fatigue, confusion, breathlessness or raised inflammatory markers may have more than one cause. The plan should define immediate observations, cultures or tests, medicine review and specialist contact without asking untrained staff to diagnose. Immunosuppression can blunt expected signs of infection.

Plan urgent organ-specific escalation

Name the preferred hospital and specialist, but do not delay emergency services solely to reach a distant centre. Decide whether the RSA can sustain the pathway
Use a real scenario involving an abnormal result or new symptom. Ask who assesses, who calls the specialist, how transport happens and how changed instructions enter the medication and care records. Verify access on evenings and weekends, not only during office hours.
Obtain written confirmation of accepted monitoring and external-treatment responsibilities. If the plan depends on relatives repeatedly flying in or interpreting clinical results, the placement is not robust. Start with a disease map written by the treating specialist. It should identify the vasculitis type, organs previously involved, damage that is now permanent and features that signalled past relapse. Kidney, lung, nerve, skin, eye and ear involvement may require different observations; a generic entry saying “vasculitis” cannot guide daily staff.
Separate relapse surveillance from infection surveillance. Fever, fatigue, breathlessness or inflammatory results may fit either, and immunosuppression can alter the presentation. The plan should tell the RSA which combinations require same-day clinical review, which samples or measurements are useful, and who decides whether treatment changes. Staff and relatives should never increase steroids or pause immunosuppression without authorised advice unless a written emergency instruction expressly covers that situation.
List every monitoring dependency: blood and urine tests, blood pressure, weight, wound or skin review, vaccination planning, prophylactic medicines and specialist appointments. For each, name the requester, result recipient and person responsible for acting on an abnormality. Ask how the facility tracks an overdue result when the hospital portal or laboratory uses a different system.
Scenario-test subtle change. If the resident develops new foot weakness, blood in urine, a painful red eye or an unexplained oxygen decline, what happens during the evening? The answer should include focused observations, a clinician contact and an emergency route. Delayed recognition can matter even when the person does not appear dramatically unwell.
Ask the specialist to distinguish activity markers from accumulated damage. Persistent foot drop, hearing loss or breathlessness may need rehabilitation and adaptation without proving active inflammation. Conversely, haematuria, rising creatinine, mononeuritis, purpura, sinus symptoms or ocular pain may require targeted investigation in the relevant vasculitis. The RSA should not interpret ANCA titres or inflammatory markers in isolation; it should preserve the testing schedule, report clinical context and route the result to the clinician who understands the phenotype.
Medication reconciliation deserves special attention when glucocorticoids, rituximab, azathioprine, methotrexate or other immunosuppressants, prophylaxis and bone or gastric protection come from different prescribers. Record purpose, cycle date, timing, surveillance and the route for interaction questions. If a dose is held because of acute illness, the decision and restart responsibility must be explicit.
Check how planned rheumatology, nephrology, respiratory, ophthalmology or otolaryngology reviews will work after relocation. Confirm referral status, transport, specimen timing and whether the RSA can transmit observations before the visit. A missed appointment may be clinically important even when the resident feels unchanged.
Make steroid toxicity visible without confusing it with vasculitis activity. Glucose instability, bruising, proximal weakness, osteoporosis, mood change and adrenal suppression may alter everyday support. Ask the prescriber which preventive measures and sick-day instructions apply, and ensure any steroid card or alert accompanies transfers. Abrupt cessation must be avoided unless an authorised plan says otherwise.
For renal surveillance, define how urine protein, microscopic blood, creatinine trend and blood pressure reach the nephrology or vasculitis team. For neuropathy, map sensation, footwear, hand function and falls; for ear, nose or airway disease, preserve hearing devices and specialist cleaning instructions. These organ-specific streams create a more discriminating record than a single generic symptom checklist.
